Sumner Mental Health Center will be transitioning to a Certified Community Behavior Health Center
Sumner Newscow report — Over the past year and the remainder of 2024, Sumner Mental Health Center will be transitioning to a Certified Community Behavioral Health Center model of care. The focus of this model is whole-person wellness and allows for care coordination among all of our community partners including physical health, alcohol and drug needs, crisis services, housing, and employment. SMHC will continue to provide traditional quality behavioral health services with the addition of patient navigation services and primary care screenings such as height, weight, and blood pressure, all in the service of integrated care.
As a CCBHC, Sumner Mental Health Center will offer same-day access for behavioral health and substance use intake assessments to improve access to services and reduce wait times. Scheduled intakes will still be available. We will use a triage system, which places priority on crisis situations. To begin with, only one intake time on the days below will be available with additional times added later if necessary. Beginning on March 25, 2024, same day access intake times are available on a first come first served basis for those who present in person:
Behavioral Health Intakes Substance Use Intakes hours:
Monday: 4:30 to 7 p.m.
Tuesday: 1:30 to 4 p.m.
Wednesday: 12:30 to 3 p.m.
Friday: 2:30 to 5 p.m.
These times ensure enough time for paperwork, meeting with the care coordinator, and completing a thorough intake interview. It is not uncommon for intakes to end before the stated end time.
Please Bring your insurance card. If you are uninsured, bring proof of income (e.g., disability, paycheck stubs, vision card, food stamps, SSI).
Children must be accompanied by a parent or guardian.
